CEI’s program is specially designed to offer an alternative to those elders who prefer to remain in their own home, but whose medical problems make it impossible for them to live at home without assistance from doctors, nurses, social workers, and other caregivers. Your care is planned and provided by an Interdisciplinary Team of specialists, working together with you. When necessary, services may also be provided in your home, in a board and care facility or other group housing, in a specialist’s office or in a nursing home. Our primary care physician will manage your hospital care. • MEDICAL SERVICES INCLUDE BUT ARE NOT LIMITED TO: 1. Physician, specialist and nursing care 2. Prescription medications 3. Outpatient labs and X-rays 4. Contracted professional services such as dentistry, optometry, audiology, podiatry, speech therapy and psychiatry 5. Durable equipment such as; walkers and wheelchairs 6. Rehabilitative and recreational therapies • NON MEDICAL SERVICES INCLUDE BUT ARE NOT LIMITED TO: 1. Activities – recreational therapy programs such as music, art, drama, celebrations, special events, exercise programs, support groups, and field trips 2. Home Care – assistance with personal care and light housekeeping 3. Meals – accommodations for special diets 4. Nutritional Counseling 5. Social Services – counseling, family support, assistance with benefits 6. Transportation to and from the PACE Centers and other services | |
